Fill out a Risk Assessment Questionnaire based on the condition, available on our website in the Underwriting menu and email or fax the questionnaire(s) back to us. You could alternately call GAIN at 800-847-6426 to discuss an impaired risk.

We recommend that you send all applications to our office so that we can scrub for missing information and inform you what’s needed. This will prevent delays in processing the application. It is mandatory to submit the 1st application for any carrier to our office so that we can get you coded under our hierarchy with the carrier. Plus some carriers do not allow for agents to submit the applications directly.
If an application(s) is submitted directly to the carrier please make sure that you mention the correct writing number on the agent page and send a cover letter to prevent delays. We also request a copy be emailed or faxed to us for follow-up.

Most carriers, including our Core Carriers do not require the original application. You can either scan and email them or fax to our office at 888-305-8151. If a check has been collected, we suggest to sill email or fax the application and only mail the check to our mailing address below.

This is not required if the full paramed is done. In case of abbreviated paramed, or another carrier’s paramed (full or abbreviated), it is mandatory to fill out Part 2 (non-med) of the application.

Lab slips and exam forms are sent directly to the carrier, but we recommend sending us copies of the completed paramed to expedite processing the case.

Yes, we recommend ordering the exams/labs and submitting the application as a packet for expeditious processing. You can alternately request GAIN to order the requirements for you when sending in the application, just write the request on you cover letter.

An Attending Physician Statement (APS) is not required for all cases. This is ordered at the discretion of the underwriter. If your client has significant medical issues, we recommend getting a copy of the medical records and submitting along with the application instead of waiting for the carrier to order it. This saves processing time.

No, motor vehicle reports, when required, will be ordered by the carrier.

It is not required for term cases, except in a few states like PA. An illustration showing desired premium payment mode is required for UL and whole life cases.

We will send you status reports via email/fax/phone based on your preference weekly, or more if updated.

For simplified issue cases, the carrier turnaround time is 4-5 days. For fully underwritten cases, it could take 2 weeks to 3 months depending on the case requirements. Obtaining medical records delays policy issue in most cases.

Most carriers mail the policies to us first, but we forward the policy same day to you.

Yes, changes can be made. If it is requested during the underwriting process, an email or fax requesting the change is required. If a change is requested after policy issue, within the Free Look Period, the policy has to be mailed back to us along with the requested change.
